On July 17, 2012, the NEW YORK STATE POLICE at Liberty, NY investigated a three-vehicle fatal accident on State Route 42 in Monticello, NY, involving a Shortline Company Bus. The operator of a 1986 AMC Jeep traveling northbound on St. Rt. 42, attempted to pass another vehicle on the right, in front of Dunkin Donuts.

While passing this vehicle he inadvertently struck it causing him to lose control of his Jeep and slid sideways into the southbound lane of Rt. 42, and subsequently strike a Shortline bus with the driver’s side of his vehicle. Prior to the collision, the driver of the Shortline bus had observed the operator of the Jeep lose control of his vehicle, and stopped the bus. The Jeep struck the bus while it was stopped. Monticello Fire Department and Mobil Medic assisted at the scene. The operator of the Jeep was transported to Catskill Regional Medical Center, where he was pronounced dead.

Six passengers along with the bus driver were treated for minor injuries at Catskill Regional, and were released after treatment. The Commercial Vehicle Enforcement Unit of the New York State Police was also at the scene and assisted in the investigation.

State Police have identified the operator of the 1986 Jeep as SAMUEL B JOSEFOWITZ, DOB 9/24/43, of Monticello, NY.
