Living Well: Chronic Disease Self-Management Program

HONESDALE, PA — Wayne County Area Agency on Aging will present a free six-week program at the Honesdale Senior Center on Fridays from 11 a.m. to 2:30 p.m. from May 23 to June 27.

Stanford University Chronic Disease Self-Management Program teaches consumers skills to manage their conditions or those of a loved one and build their self-confidence so they can be successful in adopting healthy behaviors, improve communications with their physician, and enhance their quality of life. It is designed to give older adults with chronic diseases, such as high blood pressure, diabetes and heart conditions, tips to help them make their daily life more enjoyable.

Topics include managing symptoms, fitness/exercise, nutrition, communication, medications, working with health care professionals and systems and more. Program is recommended by AOA (Administration on Aging). A minimum of 12 people needed in order to have the program.
